A data submission form and update sheet Jay Kennedy sent around when he was putting together data for the follow-up to his Official Underground and Newave Comix Price Guide from 1982 (although, sadly, never realized).
I'm uncertain of the exact year but I know he was working out the possibilities in the early to mid '90s (and maybe later).
The update sheet is 8½ x 11" and the submission form is 8½ x 14". Both are double-sided.
